Senior Independence Program
Through direct assistance programs and case management, the Senior Independence Program (SIP) fosters empowerment of neighbors age 65 and older to age in place. Senior clients are screened on a quality of living survey to assess housing, healthcare, financial, transportation, food and social interaction barriers. Clients enrolled work with a case manager to develop an individualized empowerment plan to set short-term goals to improve quality of living and connect them to resources, provide support and advocacy, and promote stabilization, as well as other support services.
This program is filled through referrals. We recommend utilizing regular assistance in Programs & Service to be considered.
This program is currently funded by grant from St. David's Foundation and Seeds of Strength and was established in 2018.
